Upon receiving an inquiry regarding care services, a member of the Angelic Home Care management team will call to set up a free, in home appointment at a time that is most convenient for you, your family and other interested parties.

Initial Meeting
During this initial meeting, the owners will meet the client and family members to talk about the needed services and how Angelic Home Care can meet those needs. This initial meeting gives the owners a feel for the best personality in an aide, we try hard to make the right match the very first time. Matching the aide with the client is a very important part of the in-home care process and making the right match is a must for success. Matching the aide with the client includes personalities, skills, expertise, interests and attitudes. Once services have been decided and agreed upon, our Director of Nursing (DON) is scheduled to do an assessment. The DON will explain Angelic Home Care services and suggest the level of service deemed most appropriate.  This assessment combined with an open, honest discussion between all parties leads to a mutually acceptable Plan of Care (POC).

Plan of Home Health Care
The Plan of Care is a document outlining the specific care services, schedules, client preferences and special instructions. Angelic Home Care employees will adhere to the POC in order to meet not only your needs, but to ensure your satisfaction.  As your situation changes, the Plan of Care is modified by the DON and notification is sent to you and anyone else you delegate. The POC provides a central focus to all parties involved with your care and ensures all of your needs are being met.

Home Care – Continued Follow-Up
After the caregiver has been placed in the home, the Angelic Home Care DON and administrators will continue to stay in touch with you and others that you may designate to ensure the best possible service is being provided. The DON will make a supervisory visit every 90 days at no charge to the client. The DON will determine if there have been any significant changes and will update the POC as needed. In addition, a daily Care Log is kept by the caregiver for the family’s review. The Care Log allows you to see what was done, when the service was done and where and how it was performed.  Our caregivers are available when you need them – seven days a week, 24 hours a day. We are dependable.  We show up when promised, and we always make sure that we go the extra mile.
